Transportation

A transport van is available for patient transportation Monday through Friday from 5:00 a.m. through 8:00 p.m.

The van will be available to transport patients within a 15-mile radius of the hospital.

Transportation will be provided for patients to come to and return home from the hospital for procedures, surgery, admission, or physical therapy. Patients may be transported from a physician office to the hospital for these reasons only.

Passengers are to be taken directly to the hospital and returned directly to their homes. The van is not to make stops for passengers at drug stores, pharmacies, physician offices, etc.

Passengers in the van must be able to care for themselves and must agree to use provided safety equipment.

Weekend transportation will be provided by a taxi service and arranged by the Nursing Supervisor, as deemed appropriate and necessary by the AHS.
